Who this is for
This is for you if you have support coordination funding in your NDIS plan and want help putting it into action. We work alongside you, your family, and your carers to find services, build your confidence, and get the best outcomes from your plan. We are experienced with complex cases and support people with a wide range of intellectual, psychosocial, and physical disabilities.
What we offer
We provide Support Coordination (Level 2) and Specialist Support Coordination (Level 3). Our experienced, multilingual team can help you:
- Connect with the right service providers and community, mainstream, and government services
- Transition from hospital, aged care, or the justice system
- Explore housing options such as public housing, private rental, or Supported Independent Living (SIL)
- Work with allied health professionals and apply for assistive technology or home modifications
- Understand service agreements, pricing, and your rights, and keep track of how your funding is used
How it works
- Get in touch — no referral form needed if you already have support coordination funding.
- We meet with you to understand your needs, preferences, and goals.
- We help you plan, explore your options, and put everything into action, working alongside you the whole way.
Can I get this?
If you are eligible for support coordination, you will find it in your NDIS plan under capacity building funding. If you, or someone you know, has been funded for support coordination, our team can help. No referral form is needed.
