Who this is for
Older people from culturally and linguistically diverse backgrounds who want to stay in their own home and keep doing the things that matter to them. Support at Home suits people who need anything from a little basic help to high-level care.
Available from our Blacktown, Mt Druitt and Penrith offices. (Penrith offers Support at Home only.)
What we offer
A range of services tailored to your needs and circumstances, including:
Case management — one point of contact to plan and coordinate your support
Help with everyday living activities at home
Care delivered by bilingual, bicultural workers who understand your culture and language
How it works
- Register with My Aged Care for an assessment (we can help you do this)
- The assessment team works out the level of care you need
- Contact SydWest and we’ll help you start your services
Can I get this?
A My Aged Care referral is required for all Support at Home services. If you’re not sure where to start, our Care Coordinators can guide you through the whole process — just call us.
