Refer yourself, a family member, or someone you support to one of our services — and we can talk it through in your language.
SydWest is a large community services organisation with a wide range of supports. You may wish to refer yourself or a client into one of them. You’re welcome to refer whether you’re a prospective client, a family member or friend, a health professional, a support coordinator, or another service provider.
How to refer
If you’d like to refer a client or discuss the options with one of our team, complete the online referral form. We’ll ask a few questions about you and the person being referred, and a copy of the completed form will be sent to your email address.

Aged Care Services
We help people live independently in their homes and in the community for as long as possible, with a focus on people from culturally and linguistically diverse backgrounds.
Disability Support Services
As a registered NDIS provider, we deliver quality support to people with disabilities, their families, and their carers.
Families Support Services
We help parents and families from culturally and linguistically diverse backgrounds raise happy, healthy families — supporting parents who are expecting a baby and families with children up to eight years of age in the Blacktown and Hills areas.
Youth Services
A fun, comprehensive youth program for young refugees, humanitarian entrants, and migrants settling into Australia.
Settlement Services.
Support for newly arrived refugees and humanitarian entrants (less than five years) to help them settle into the Australian way of life.
