Talking to your family and loved ones about your wishes for the future healthcare is important, even if you do not write them down.
If you do create an Advance care directive (care directive) be sure to share it with your health care team such as GP and anyone else you want access to it.
Let your family and loved ones know that you have a care directive and where to find it. It’s also important to review your plan regularly- maybe on your birthday- to make sure it still reflects your wishes.
Remember, a care directive is a way to ensure that your family and health care professionals consider your healthcare and treatment preferences if you are unable to communicate them yourself.
