Who is eligible?
You may qualify for our aged care services if you’re an older person from a culturally diverse background who needs some assistance to keep living at home. Our services are funded by the Department of Health.
What you can expect from us
- Care Coordinators who guide you through the registration and assessment process
- Fully qualified, bilingual and bicultural care workers — you can choose a worker who speaks your language and understands your culture
- A personally designed program delivered with dignity, confidence and independence in mind
How funding works
- To access government-funded support, register with My Aged Care for an assessment, or call 1800 200 422
- The assessment decides what level of support you’re eligible for
- We help you put that funding to work through the right SydWest services
Schedules of fees are available for Support at Home, group-based services and CHSP — ask us for the current versions.
