A plain-language guide to who can access our aged care services and how government funding works.

 
 

Who is eligible?

You may qualify for our aged care services if you’re an older person from a culturally diverse background who needs some assistance to keep living at home. Our services are funded by the Department of Health.

What you can expect from us

  • Care Coordinators who guide you through the registration and assessment process
  • Fully qualified, bilingual and bicultural care workers — you can choose a worker who speaks your language and understands your culture
  • A personally designed program delivered with dignity, confidence and independence in mind

How funding works

  1. To access government-funded support, register with My Aged Care for an assessment, or call 1800 200 422
  2. The assessment decides what level of support you’re eligible for
  3. We help you put that funding to work through the right SydWest services

Schedules of fees are available for Support at Home, group-based services and CHSP — ask us for the current versions.
