At SydWest Multicultural Services, we provide quality approved aged care services to assist people from culturally and linguistically diverse backgrounds to live independently – at home and within the community. You can access our aged care services from all our offices in Blacktown, Rouse Hill and Mt. Druitt.

Services

Home care packages

Home Care Packages support you to live independently in your own home for as long as possible. SydWest Multicultural Services offers a range of support services tailored to your individual needs and circumstances. The programs are suitable for people who require basic to high-level care and the services include case management and assistance with activities of daily living.

Commonwealth home support program

Ongoing or short term support services can be provided through the Commonwealth Home Support Program and can include; help with housework, assistance with showering and personal grooming, social support and respite (which, if required may include preparation of culturally appropriate light meals).

Social support groups

Our regular social support groups for seniors help break social isolation through social outings, information sessions and group sharing. Current groups include Coptic/Arabic, Bhutanese, Chinese, Croatian, Filipino, Indian/Fijian Subcontinent, Indian Glenwood, Maltese, Serbian, Spanish, Turkish, Iranian, Sri Lankan, Blacktown Multicultural and Mt Druitt Multicultural seniors’ groups.

We also offer a centre based respite program that provides day care for seniors

Healthy active living

SydWest Multicultural Services is proud to have funded and was part of the pioneers of the Healthy Older People Partnership (HOPP) program which was piloted in 2016 for elderly clients from non-English speaking backgrounds living Western Sydney. We continue to offer this program to our clients. We provide trained Care Workers who teach our aged care clients the key exercise activities aimed at improving strength and balance and can help reduce the risk of falls in the home.

Eligibility

You may be eligible for support from SydWest if you are an older person who needs coordinated services to help you stay in your home.

You may be eligible for support from SydWest Multicultural Services if you:

  • Are an older person who needs coordinated services to help you to stay in your home (can be younger in certain circumstances, if approved by Aged Care Assessment Team)
  • Need assistance to complete everyday tasks
  • Are from a culturally and linguistically diverse background

Our care workers

All SydWest Multicultural Services Care Workers are fully qualified and trained.

They are also bilingual and bi-cultural, covering a range of cultural backgrounds and languages represented in Western Sydney.

You are able choose a Care Worker who speaks your language and understands your culture.

Applying for a government funded aged care service?

To access funded support services, individuals must register with MyAgedCare for assessment (www.myagedcare.gov.au or 1800 200 422). If you require assistance to complete this process please contact a member of the Aged Care and Disability Services team on (02) 9621 6633 to discuss your needs.

Our experienced team can help you navigate the application processes for Aged Care Services.

2.Level of care assessment

The assessment team will determine what level of care you require and send you a copy of the assessment.

3.Commencing support services

Once you receive the outcome of your assessment please contact SydWest and one of our friendly staff will help you to commence your support services.
